Patient Assessment Form and Questionaire
We ask our new patients to arrive at their appointment 10 minutes early to complete a new patient assessment form. The form includes a questionnaire on your teeth, general health and wellbeing.
Initial Consultation
Your new dentist will discuss any concerns that you have with your oral health. Perhaps you are suffering from discomfort or you are unhappy with your smile? Your dentist will be happy to talk you through the options available to meet your needs.
Oral Examination
During your first appointment, your new dentist will conduct a thorough examination of your teeth, gums, bite, and jaw joints. The examination also includes an oral and throat cancer check. To be thorough, we usually take some X-rays and/or photographs of your mouth. The images will help your dentist discuss the state of your teeth, gums and jaw with you after the examination. Everyone is different and we take the time to listen to their wants and needs.
How to Prepare for Your First Dental Appointment
Bring Your Dental Information
Bring details of your medical history, current medications, and any relevant dental records or X-rays you may have.
Arrive a Little Early
Allow some extra time before your appointment to complete any required forms and settle in before your visit.
Share Any Concerns
Let the dental team know about any pain, sensitivity, dental anxiety or specific concerns you would like to discuss.
